In a pre-print copy of the work I have, the copyright is mentioned to belong to “Open Source Press”. Working from the distance, I don’t have access to my archives currently. Digging mails from 2010/2011 is of course an option too.

England and Wales: I’m a bit reluctant to use BY-SA as it does make it more difficult to quote parts of the book in other works. I’d be happy with CC-BY (but if the consensus is CC-BY-SA, then fine. Ultimately, since I think the Ur-chapter which we all based ours on was written by Ywein (correct me if I’m wrong), all chapters will ultimately be driven by his decision anyway)).
